Service of Suit Endorsement
This Endorsement Changes the Policy. Please Read It Carefully.
This endorsement modifies insurance provided under the following:
The Company hereby appoints the Superintendent, Commissioner or Director of Insurance or other officer specified for that purpose in the statute or his successor or successors in office, as the agent upon whom may be served any lawful process in any action, suit or proceeding instituted by or on behalf of the “Insured” or any beneficiary hereunder arising out of this contract of insurance.
The Company furthermore designates CT Corporation System, 208 South LaSalle Street, Suite 814, Chicago IL 60604, as the agent to whom a copy of the Service of Process should be forwarded by the Superintendent, Commissioner or Director of Insurance or other officer specified for that purpose in the State of AZ.
